Appointment Information

Patients are seen by appointment only. Please bring your insurance card and photo ID to each visit.

New Patients

All new patient’s first office visit will be scheduled as a comprehensive new patient appointment to review ongoing medical conditions, health history, medications, and any health concerns you may have. Please bring any past medical history and actual medication bottles/dispensers to your visit.

Established Patients

When scheduling an appointment, please give the receptionist as much information as possible to ensure you are scheduled an appropriate amount of time.

Late Arrival/Cancellation Policy

If you arrive 10 minutes after your scheduled appointment time, you may be asked to reschedule depending on the provider’s schedule for that day. We require a 24-hour notice for all cancellations. A cancellation fee of $60-$100 may apply. No show appointments may be subject to a cancellation fee of $60-$100. This charge will be your responsibility; your insurance carrier will not be billed.

After Hours Patient Services

Our main office number, 817-481-4739, is answered between normal business hours. After hour calls are forwarded to the physician on-call. In a life-threatening situation call 911.

Medication Refills

When you need a refill for medications, requests must be made through the Your Health File Portal. Requests are not processed after office hours, weekends or holidays. Please allow 24 hours to process refill requests. 

A refill request will be denied if you missed a scheduled appointment, are not current on any laboratory tests required for the medication, or have not had your annual physical exam.

An annual physical is required on every patient with a medical condition that is treated in our office

Payment Policy

 We are contracted with many insurers and health plans. We will bill those plans with which we have an agreement. We will collect any required co-payment, deductible or co-insurance amount based on information provided to us from your insurance company at time of service. It is patient responsibility for ensuring that we are an in network providers and for knowing what services you have coverage for.
